HOW TO REFER A PATIENT TO SENSEWELL PSYCHOLOGY:

SenseWell Psychology is a Medicare Service Provider so Mental Health Treatment Plans can be accepted for eligible clients. To make a referral please download the referral form on this page and return it to us by email at info@sensewell.com.au, along with a Doctor's Letter of Referral and the Mental Health Treatment Plan.

SENSEWELL PSYCHOLOGY FEEDBACK TO HEALTH PROFESSIONALS: 

​

SenseWell Psychology sends referring Practitioners (GP's, Psychiatrists and Paediatricians) a referral acceptance letter, a six week review about the client's progress and a follow up review when the patient has been discharged from our Services. Each report will include information about the presenting issues, treatment provided, goals achieved and being worked on and clinical recommendations. SenseWell Psychology sees reporting back to referring Health Professionals as an important opportunity for effective collaborative patient care.
